Pledge Sheet
Use this sheet to track your pledges. It is for your records only and does not need to be turned in.
Receipts
Give to donors who support your cycling efforts. Donations for $250 or more will receive a tax letter from the MS Society.
Adult Waiver & Emergency Form
Print, read, fill out and bring with you to Friday Fuel-Up. Participants will not be able to ride without a waiver.
Youth Requirements
Bike Inspection Form
Take this form to your local bike shop to make sure your bike is ready for ride day! Bring this with to Friday Fuel-Up.
